New Patients

New Patient Consultations

All new patients do a consultation with Dr. Goodfellow. Consultations are done in two parts. The first part is a video call that takes between 30-45 minutes and has a fee of $150 (dental insurance code: 08012) and the second part is in the office and takes between 60-90 minutes and has a fee of $330 (dental insurance codes: 01204 – $160; 93123 – $40; 05104 – $50; 04813 – $80). Our patients pay us directly and we provide them with a receipt. We do not direct bill insurance companies. Our services are not covered by OHIP. Fees for TMJ treatment range from $2900 to $6000.

As preparation for the video call, we have an intake form that you would need to fill out prior to your call. You will receive the forms after you schedule an appointment.
