TMJ & Sleep Therapy Centre of the Greater Toronto Area (“we”, “us”, “our”, “the Centre”) is committed to protecting the privacy of our patients and website visitors. This Privacy Policy explains what information we collect through this website, how we use it, who we share it with, and the choices available to you.
We handle personal information in accordance with Canada’s Personal Information Protection and Electronic Documents Act (PIPEDA), Ontario’s Personal Health Information Protection Act, 2004 (PHIPA), and Canada’s Anti-Spam Legislation (CASL). Where visitors access our website from the European Economic Area or the United Kingdom, we also apply the standards of the General Data Protection Regulation (GDPR).
1. Scope of this Policy
This Policy applies to information collected through tmjsleeptreatmentcentres.com.
Your clinical records — the personal health information created and maintained in the course of your care at the Centre — are governed separately by PHIPA and by our in-clinic privacy practices. Our Health Information Custodian is responsible for those records. If you would like a copy of our clinical privacy practices, please contact us using the details in Section 13.

We collect information you voluntarily submit through forms on this website, including:
- Appointment requests — your name, phone number, email address, and any details you choose to share about your symptoms or reason for enquiry
- Contact forms — your name, contact details, and message
- Referral forms — referring practitioner details, patient name and contact information, and clinical background provided by the referrer
- Correspondence — any information you include when you email or telephone us
Where a website form asks about symptoms or health concerns, the information you provide may constitute personal health information and is treated accordingly.
2.2 Information collected automatically
When you visit our website, certain information is collected automatically, including:
- IP address and approximate geographic location
- Browser type, device type, and operating system
- Pages viewed, time spent on pages, and scroll depth
- The website or advertisement that referred you to us
- Links and buttons you click, including click-to-call actions
2.3 Telephone call information
We use call tracking to understand which marketing activities lead patients to contact us. When enabled, this technology may display a different telephone number to different visitors and may record the date, time, duration, and originating number of calls. Calls placed to the Centre may be recorded or monitored for quality and training purposes; where this applies, you will be notified at the start of the call.

3.1 Your cookie choices
When you first visit our website you will be presented with a cookie notice. Depending on your location, you may be asked to consent before non-essential cookies are placed, or given notice with the ability to opt out.
Where consent is required, statistics and marketing technologies are held in a denied state until you accept them, and no advertising or analytics identifiers are set. You may change or withdraw your preferences at any time using the cookie settings link in the footer of every page.
You can also control cookies through your browser settings. Please note that blocking all cookies may prevent parts of the website — including our appointment request forms — from working correctly.
3.2 Do Not Track
Our website does not currently respond to browser “Do Not Track” signals, as no common standard for interpreting them has been adopted.

5. Consent
We collect, use, and disclose personal information with your knowledge and consent, except where permitted or required by law. Your consent may be express — for example, when you submit a form or tick a checkbox — or implied, where the purpose is obvious and you voluntarily provide the information.
You may withdraw your consent at any time, subject to legal and professional record-keeping requirements, by contacting us using the details in Section 13. Withdrawing consent may limit our ability to provide certain services to you.
6. Disclosure of Information
We do not sell your personal information. We do not rent or trade patient information for the marketing purposes of others.
We may share information in the following circumstances:
- Service providers — website hosting, email delivery, form processing, call tracking, and analytics providers who process information on our behalf under contractual confidentiality and security obligations
- Within your circle of care — with referring physicians, dentists, sleep physicians, and other health practitioners involved in your treatment, in accordance with PHIPA
- Insurers and payors — where you have authorised us to submit claims or provide supporting documentation
- Legal and regulatory — where required by law, court order, or a governing professional body
- Business transitions — in connection with the sale or transfer of the practice, subject to equivalent privacy protections and any applicable notice requirements
7. Advertising and Analytics Providers
Our website uses services provided by Google LLC (Google Analytics, Google Tag Manager, Google Ads, reCAPTCHA, and Google Maps) and CallTrackingMetrics. These providers may set cookies and process technical information, including IP address, in accordance with their own privacy policies.
Where consent is required and granted, we transmit consent signals to Google so that these services operate in a privacy-respecting mode consistent with your choices. Where consent is declined, advertising and analytics identifiers are not set.
You can opt out of Google Analytics across all websites using the browser add-on available at tools.google.com/dlpage/gaoptout, and manage Google advertising personalisation at myadcenter.google.com.
8. Storage, Transfers, and Location of Information
Information submitted through this website may be stored or processed on servers located outside Canada, including in the United States. Where information is held in another jurisdiction, it may be accessible to the courts, law enforcement, and national security authorities of that jurisdiction.
We take reasonable steps, including contractual safeguards, to ensure that service providers protect your information to a comparable standard. If you have questions about where your information is held, please contact us.
9. Electronic Communications
Where we send commercial electronic messages, we do so in accordance with CASL. Every such message includes a clear method of unsubscribing, and we act on unsubscribe requests promptly. Messages relating directly to your care, appointments, or an enquiry you initiated are not commercial electronic messages and will continue to be sent as necessary.
Please note: standard email and website forms are not secure methods of transmission. We ask that you do not send detailed personal health information by email. If you need to share sensitive clinical information, please contact the clinic by telephone so we can arrange a secure method.
10. Retention
We retain website enquiry information only as long as necessary for the purposes described in this Policy, or as required by law.
Patient health records are retained in accordance with the requirements of the Royal College of Dental Surgeons of Ontario and applicable Ontario legislation. Analytics and advertising data are retained according to the retention settings of the relevant provider.
11. Security
We maintain administrative, technical, and physical safeguards appropriate to the sensitivity of the information we hold. These include encryption of the website in transit (TLS), access controls, multi-factor authentication for administrative accounts, malware monitoring, and staff privacy training.
No method of transmission or storage is completely secure, and we cannot guarantee absolute security. If we become aware of a privacy breach that poses a real risk of significant harm, we will notify affected individuals and the appropriate regulator as required by law.

Subject to certain legal exceptions, you have the right to:
