New Patients

New Patient Consultations

All new patients do a consultation with Dr. Goodfellow. Consultations are done in two parts. The first part is by video and takes about 45 minutes and has a fee of $180 (dental insurance code: 08012) and the second part is in the office and takes approximately 60-90 minutes and has a fee of $400 (dental insurance codes: 01204 – $190; 93123 – $60; 05104 – $65; 04813 – $85). Our patients pay us directly and we provide them with a receipt. We do not direct bill insurance companies. Our services are not covered by OHIP. Our treatment fees for TMJ and sleep-breathing disorders range from approximately $3700 to $7600.

We also offer emergency appointments (fee: $200, plus the cost of any temporary devices needed; dental code: 01205). The emergency appointment is meant for short-term relief and is intended for patients in severe pain and/or struggling with day-to-day functions, such as eating. It is not intended to be a long-term solution, as our full treatment plans are. After the emergency appointment, you may continue on in our treatment protocol to the second part of our consultation (in office) (bypassing the first video consultation).

As preparation for the video call, we have an intake form that you would need to fill out prior to your call. You will receive the forms after you schedule an appointment.
